Prepared |
When you
are ready to apply, please be prepared
to provide the following application and
property information. |
Application Information
(for all applicants) |
Your home address(es) for the previous two years
(A) If you own your home we need property address, current
market value, mortgage lender name, account number, current monthly
mortgage payment, and outstanding mortgage balance.
(B) If you are renting we need the property address, name and
address of landlord, current monthly rent, and pervious
address/landlord if you've lived in your current address for less
than two years.
Your
social security number(s)/Date of Birth
Employment
information for the previous two years including employer name,
address, and phone number
Income
information including salary, overtime, bonuses, commissions,
dividends, interest, retirement and any source of ongoing income.
Alimony, child support, or separate maintenance income need not be
revealed if you do not wish to have it considered as a basis for
repaying this obligation.
Account
Information
including bank name, account type, balance, and source of
down payment
Current assets/Personal
Property including the value of IRAs, CDs, bonds, stocks, life
insurance, retirement funds, vehicles, jewelry, etc.
Liabilities
including creditor names and outstanding balances for all debts
including notes payable, 401(k) loans, life insurance loans, stock
pledges, alimony, child support, co-sign loans, credit union loans,
and other liabilities
Real
estate owned including property address, market value, outstanding
liens, and rental income, mortgage payments, taxes, insurance and
maintenance dues |

property, the legal description of the property, and receipts for or
down payment deposits.
